Without entering the link I already knew it was gonna be the live version with Mikael lol. A shame he didn't make it on the studio version. There's also a version where Portnoy actually growls and while not better than Mikael's it's far better than what we ended up getting

I agree. Both this song and "The Count of Tuscany" are near-perfect songs, marred by very specific stylistic choices that involve Portnoy's unpolished vocal delivery. But they're each so ridiculously good that I accept the imperfections.

I remember not liking this song that much upon first listen. Yeah, Portnoy's cosplay as Akerfeldt was cheesy, and I felt like that opening riff was trying too hard to be Dimmu Borgir. But man, time has been very kind to this track. I love it more and more -- especially the "medication" theme. If they had brought that back at the end, with a heavier tone, this song would now be perfect.

I don't consider it disrespectful at all. Dream Theater is famously known for paying direct homages to their influences, most notably the song "Octavarium," which is a stylistic nod to the bands that came before them. I consider this specific quality as a major reason they have been so successful, much more so than Queensryche, Watchtower, or Fates Warning, who all predate Dream Theater in creating today's progressive metal genre.
